Applying for ALTCS

How to apply for ALTCS in Arizona, step by step

What to gather, what the Pre-Admission Screening actually looks for, how long approval takes, and what families do while waiting.

Before you start: gather these documents

The financial review moves faster when the paperwork is ready before you apply, not scrambled together after a request letter arrives. Collect, for the applicant:

  • Social Security card, birth certificate, and photo ID
  • Proof of all income — Social Security award letters, pension statements, annuity statements
  • Bank, investment, and retirement account statements going back 60 months
  • Life insurance policies, including cash value
  • Property deeds and vehicle titles
  • Burial contracts, burial insurance, or prepaid plots
  • Marriage certificate and, if applicable, divorce decree
  • Medicare and any private insurance cards
  • Recent hospital discharge summaries and a current medication list
  • Physician contact information and recent medical records

If a transfer of money or property happened anywhere in the last five years — a gift to a grandchild, a car given to a caregiver, a house sold to a relative below market value — bring documentation of it. It will surface in the look-back review regardless.

The application, step by step

  1. Apply. Apply through Health-e-Arizona Plus online, by calling AHCCCS on 1-888-621-6880, or in person at an ALTCS office. There is no cost to apply.
  2. Complete the PAS assessment. An assessor evaluates functional and cognitive need in person. This determines whether the applicant meets Nursing Facility Level of Care.
  3. Submit the financial documentation. Income and assets are verified against the limits, and the 60-month look-back is applied to any transfers.
  4. Respond to any follow-up requests promptly. Missing documents are the single most common cause of delay. A caseworker will ask for specifics — answer completely and quickly.
  5. Receive a decision. Approval commonly takes up to about 90 days, though straightforward cases can move faster.
  6. If approved, enroll with a managed care plan and begin working with an assigned case manager, who calculates any share of cost and coordinates ongoing services.

How the PAS assessment actually works

The assessment is usually done through an interview and observation, either in the home, in a hospital, or wherever the person currently is. The assessor asks about daily function directly and also talks to family or caregivers who see the person day to day.

Answer honestly about the person’s worst days, not their best ones. This is the most common way families accidentally work against themselves. Someone who can dress themselves once a week with a lot of coaxing, but needs full assistance most days, is not accurately described as independent with dressing. Describe what actually happens on a typical day, including the bad ones, and bring a family member who has seen it firsthand if you can.

Being discharged from the hospital today?

If a discharge date has been set and the ALTCS application is not filed yet, call us on (307) 200-5270 before you do anything else. We work these situations constantly and can often help line up an ALTCS-pending placement while the application is still moving through the system, so a discharge date does not force a placement no one had time to think through.

What to do while you wait: ALTCS-pending placement

Ninety days is a long time to wait when someone needs care now. Most families cannot simply pause. This is where ALTCS-pending placement matters — moving into a home while the application is still under review, rather than after approval.

Not every home will accept an ALTCS-pending resident, because the home is taking on the risk that the application could be denied. Homes that do generally use a temporary private-pay arrangement, sometimes with a retroactive adjustment once ALTCS approval comes through and coverage is applied back to an earlier date. Knowing which homes currently do this, and on what terms, changes week to week — it is one of the main things we track for families.

After approval

Once approved, the member is enrolled with an ALTCS managed care plan and assigned a case manager, who becomes the ongoing point of contact for authorizations, service changes, and any share-of-cost recalculation. Approval is not the end of the process — it is the start of an ongoing relationship with that case manager, and staying responsive to their requests keeps services running smoothly.

Please note. This page is general information, not legal, medical, or financial advice. Eligibility determinations are made solely by AHCCCS. Heaven Sent Senior Placement is a placement and referral service, not a healthcare provider. For asset planning ahead of an ALTCS application, consult an Arizona elder law attorney.
