For care home owners and administrators
We refer residents whose care needs, payer and personality fit what your licence and your team can actually support, so the placement holds for you as well as for the family.
Stephanie reviews the medication list, mobility, cognition, behaviours and payer before she calls you. You hear about residents your licence level and staffing can serve, and you tell us what you accept, so we don’t waste your time.
We give every family the written disclosure A.R.S. § 36-446.14 requires before referral, and send you proof of the disclosure and acknowledgement within 14 days of your Move-In Notice.
Our fee applies to private-pay move-ins only. There is no fee on ALTCS/AHCCCS, VA or other government-funded residents, hospital-funded residents awaiting ALTCS, or skilled nursing placements.
What we ask of you
A referral network is only as good as its last update. We will check in with you regularly about open beds, rates and what you accept. When something changes, tell us and we stop sending referrals that no longer fit.
