For care homes

How a referral works.

Plain words on the process and the fee. The signed Community Referral Fee Agreement governs; this page is a summary.

  1. We call you first. Before giving a family your name, Stephanie checks the resident’s needs and payer against what you have told us you accept, and confirms you have a bed.
  2. The referral is made when we first give you the client’s name or bring the family to tour. If you already had documented contact with that family in the previous 90 days, tell us in writing inside the 48-hour window the agreement allows.
  3. The family receives our disclosure, as A.R.S. § 36-446.14 requires, before or at the time of referral.
  4. You decide on admission. Assessments, admission and care are always yours.
  5. Send a Move-In Notice within 14 days of move-in: the date, the first month’s rent and care charges, and the payer.
  6. We invoice from that notice and send you proof of the family’s disclosure and acknowledgement within 14 days.

The fee

Our standard fee is 100 percent of the resident’s first full month of rent and care charges (base rate plus level-of-care, medication management and second-person charges, before move-in discounts), due within 30 days of move-in. Deposits and one-time community fees are excluded. For some referrals we agree a different payment schedule in writing before the referral, for example split across the first two months.

When there is no fee

  • Residents funded in whole or in part by ALTCS/AHCCCS, Medicare, Medicaid, the VA or any other government health care programme
  • Hospital-funded residents awaiting ALTCS
  • Skilled nursing placements

Fair dealing, both ways

Our right to the fee lasts 12 months from referral. If a resident leaves within the first 30 days for reasons other than your own discharge or notice, we credit the unused days against your next placement fee. Either party can end the agreement with 30 days’ written notice.
