Required disclosures

Arizona referral agency disclosures

Arizona law requires senior referral agencies to disclose how they are paid and any ownership interest they hold. We publish it.

Ownership disclosure

Stephanie Acosta, our senior placement advisor, holds an ownership interest in an Arizona assisted living home. We do not refer clients to it. If that ever changes, we will tell you in writing, with the fee arrangement, before any referral or tour, as A.R.S. § 36-446.14 requires.

Real estate disclosure

Ryan Moxley, our founder, is a licensed Arizona real estate agent. If a family we work with also needs to sell or buy a home, we will tell you about that relationship in writing before any real estate referral, and you are free to use any agent you choose.

What this means in practice

  • You never receive an invoice from us. The home pays our fee from its own funds, after move-in.
  • Before you move in, we tell you in writing the fee the specific home you choose will pay us.
  • We take no fee at all on placements funded in whole or in part by ALTCS/AHCCCS, the VA or another government health care programme, on hospital-funded placements awaiting ALTCS, or on skilled nursing placements.
  • We present options that pay us nothing when they are the right fit.
  • You can end our involvement at any time, in writing, without cost.

What we are not

Heaven Sent Senior Placement is a placement and referral service. We are not a healthcare provider, and we do not deliver nursing or medical care through this business. Stephanie holds a bachelor’s degree in nursing. She is not a licensed nurse and does not provide nursing or medical care. Information on this website and in conversation is general information, not medical, legal or financial advice.

Eligibility for the Arizona Long Term Care System is determined solely by AHCCCS. We help families understand and navigate the programme; we cannot approve, guarantee or expedite an eligibility determination. Medicaid asset planning is the practice of law, so please consult an Arizona elder law attorney. We receive no compensation for attorney referrals.

Licensing of the facilities we refer to

Assisted living homes and centres in Arizona are licensed and inspected by the Arizona Department of Health Services. We encourage every family to review a prospective home’s licence status and inspection history directly with ADHS before moving in, and we will help you do it.

Questions or concerns

Call (307) 200-5270 or email heavensentplacement@gmail.com. If you believe a referral agency has violated Arizona law, you may contact the Arizona Attorney General’s Office.
