How we’re paid

Free to your family. Here is exactly who pays us and how much.

Almost every placement agency in Phoenix leads with the word “free.” Most of them never explain the mechanism. The mechanism is simple, it is legal, and you are entitled to know it before you take a single recommendation.

The short version

When your loved one moves into a community or home we referred you to, that community pays us a referral fee, typically 50 to 100 percent of the first month’s rent and care charges. Our standard fee is 100 percent; a few homes pay a lower agreed amount. Before you move in, we tell you in writing the exact fee the home you choose will pay us. You pay us nothing, and our fee does not increase your rent or your rate.

What Arizona law requires of us

Arizona regulates senior referral agencies under A.R.S. § 36-446.14. Before or at the time we refer you, we must disclose in writing:

  • Any business relationship, common ownership, or control we have with the facility
  • That the facility pays us a fee
  • The amount of that fee, or a good-faith estimate of it

We must give you that disclosure in fourteen-point type and ask you to acknowledge receiving it. You may end our services at any time in writing. Our standing disclosure is published here — you should not have to ask for it.

The conflict of interest, named out loud

A service paid by the places it recommends has an obvious incentive problem. Pretending otherwise would be insulting. Here is how we handle it.

  • We show you options that do not pay us. ALTCS placements frequently carry little or no fee. They are a large part of what we do.
  • We disclose Stephanie’s interest in a care home. Stephanie holds an ownership interest in an Arizona assisted living home. We do not refer clients to it.
  • We tell you when you don’t need us. Sometimes the answer is home care, or a family arrangement, or waiting. We will say so.

What you get, at no cost

  • A clinical assessment of the level of care actually required
  • A short list of real openings that accept the acuity and the payer
  • Tours, with Stephanie present
  • Help understanding ALTCS, VA benefits, and long-term care insurance
  • Follow-up after the move, and relocation if the placement is wrong

What we are not

Heaven Sent is a placement and referral service. We are not a licensed healthcare provider, we do not deliver nursing care, and nothing on this site is medical, legal, or financial advice. For decisions about Medicaid asset planning, speak with an Arizona elder law attorney — we can point you to several and we are not paid for those referrals.

Tell us what’s happening. We’ll tell you what’s possible.

A conversation with Stephanie Acosta costs nothing and carries no obligation, including if the answer is that you don’t need us yet.
