Trust

Compliance, in plain words.

What Arizona law requires of a placement agency, and what we do about it.

A.R.S. § 36-446.14

We are compliant with A.R.S. § 36-446.14. Every family receives our written disclosure, in 14-point type, before or at the time of referral, and we ask for a signed or electronic acknowledgement. The disclosure states that the receiving home pays our fee, the typical fee range (50 to 100 percent of the first month’s rent and care charges) and the family’s right to end our services at any time. When a resident moves in, we send the home proof of that disclosure within 14 days of its Move-In Notice, and we keep the records for at least a year. Read our published disclosures.

Fees on government-funded care

We accept no fee from a home for any resident funded in whole or in part by ALTCS/AHCCCS, Medicare, Medicaid, the VA or another government health care programme, for hospital-funded residents awaiting ALTCS, or for skilled nursing placements.

Information we collect, and what we don’t

  • Our referral form asks only for patient initials, age, area, payer, care needs and the referrer’s contact details. Never a name, date of birth, medical record number or room number.
  • Every free-text box carries a reminder not to include names, dates or record numbers.
  • Health and personal information about a family is held only after the family signs our Client Placement Agreement, which authorises us to share what is needed to find a placement.
  • Hospitals and clinicians should send records only through their organisation’s approved secure channel.

What we are not

Heaven Sent Senior Placement is a placement and referral service, not a healthcare provider. ALTCS eligibility is decided by AHCCCS alone, and Medicaid asset planning is a question for an Arizona elder law attorney.

Insurance

TODO: insurance details once the general and professional liability policies are bound.
